Most people complete our secure registration online—it's quick, confidential, and gives us the info we need to get to know you: your health history, medications, and any concerns you’d like to discuss at your first visit.
Prefer to do it in person? No problem.
Just stop by during clinic hours, and we’ll walk you through it step by step. We’re happy to help you get registered in whatever way works best for you.
Once you're registered, we’ll reach out to schedule your first visit and officially welcome you to Mahler Family Medicine.
